What services can you access with Care Connect?
Care Connect offers a broad range of in-home services to help people live safely, happily and independently at home. From personal care and nursing through to transport, allied health, domestic assistance and social support, we'll connect you with the services that best meet your needs. These services are available through Support at Home, the Commonwealth Home Support Program (CHSP) and the Out of Hospital Care program, delivered by trusted local care workers and providers in your community.

My Aged Care determines your eligibility, not Care Connect. The general criteria are being 65 or older (50 or older for Aboriginal and Torres Strait Islander people), an Australian resident, and having been assessed as needing support at home. To begin, contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au. Our team can walk you through the process by calling us on 1800 692 464.

Start time depends on how quickly your My Aged Care assessment can be arranged and approved, and then also depends on the government funded program that is relevant to you. You can be approved for funding but then need to wait up to 12 months for the funding to be assigned. Wait times can very dependng on your personal circumstances. Yes. You have the right to choose your provider and can switch at any time. Care Connect can help you transfer and ensure a stress-free process. We will manage the paperwork and coordinate the transfer with My Aged Care.
